CMS' Care Management Services - Implementation Guidance This update includes details for the new (as of Jan 1, 2024) monthly care management codes and services for Community Health Integration (CHI) and Principal Health Navigation (PIN). We consider these virtual services a vital expansion of CMS’ care management services that support individuals in meaningful ways between visits; these codes also provide pathways to reimbursement for community health workers (CHWs) and peer specialists.
