Rural Health Transformation Program
The Rural Health Transformation (RHT) Program will empower states to strengthen rural communities by improving health care access, quality, and outcomes by transforming the health care delivery ecosystem. The Centers for Medicare & Medicaid Services (CMS) will award $50 billion in RHT funding over 5 fiscal years. Uses of funds include promoting consumer-facing, technology-driven solutions for the prevention and management of chronic disease and providing training and technical assistance for the development and adoption of technology-enabled solutions that improve care delivery in rural hospitals.
